Re: Medical Requirements for Enrollment A. Y. 2026-2027

As per Memorandum Order No. 05-0437, series of 2026, the following are the medical requirements for all qualified incoming first year students of A. Y. 2026-2027.

✅ Chest X-ray
✅ Complete Blood Count (CBC)
✅ Urinalysis (Urine Exam)
✅ Fecalysis (Stool Exam)

🔔 To those students who already secured their Laboratory/Diagnostic Tests Requests last May 26, 2026, kindly take note of the changes.

🔔 To students who have not yet secured their lab requests, visit the CAS Dean's Office on June 1, 2026, onwards.

Once your laboratory tests are complete, proceed to the ASU Health Services Unit for the issuance of Medical Certificate on June 10, 2026, onwards (first-come, first-served basis).

Bring the following requirements:
1. Original and one (1) photocopy of complete laboratory results
2. One (1) short envelope
3. Face mask
4. Ballpen
